Immunization requirements for the 2026–2027 School Year

If you have a student in one of these 3 categories submit an updated South Carolina Certificate of Immunization listing all required vaccines to the school nurse ASAP

  1. Entering Preschool: Must be up to date based on their age. This is REQUIRED to attend preschool.

  2. Entering Kindergarten: In addition to mandatory immunizations for preschool, ensure your child has received (2) doses of hepatitis A vaccine received on or after the first birthday and separated by at least 6 months and (2) doses of both MMR and Varicella. This is REQUIRED to start Kindergarten.

  3. Entering 7th Grade: (1) dose of Tdap vaccine on or after their 10th birthday. This is DIFFERENT from the Dtap vaccine! This is REQUIRED to start 7th grade.

–Parents/guardians must bring or email to the school nurse Christy Thompson (christythompson@montessoricharterschool.com) a legible copy of the student’s updated SOUTH CAROLINA CERTIFICATE OF IMMUNIZATION. Request this record from your child’s doctor or print a copy form the SIMON state immunization database. Do not submit doctor’s office records or an individual immunization record, schools must have an updated SC CERTIFICATE OF IMMUNIZATION form 4024 or SC IMMUNIZATION EXEMPTION on file for EVERY student.

–Instructions on accessing your child’s immunization records in SIMON:

1) Go to the SIMON Portal https://dph.sc.gov/ImmunizationRecords. Follow the steps listed. There will be several forms available to print, however the South Carolina Certificate of Immunization Record (DPH Form 4024) is the only valid record for school. 

***Non-compliant students will not be eligible to begin school until proof of required vaccination is provided on a South Carolina Certificate of Immunization form or an exemption is on file. This is State Law! If you are transferring from out of state the Department of Health or a SC doctor can enter your child’s out of state immunization records into SIMON and provide you a copy. Medication At School

If your child will need Epinephrine, Albuterol inhaler, or emergency seizure medication at school, indicate this on the “School Nurse Health Information/Emergency Card” when registering your child. Parents/guardians are responsible for obtaining the medication and doctor’s order for the current school year and signing it in with the school nurse August 3-7, 2026. There will be additional paperwork to sign at that time. Medication should not expire during the school year. 

 ~Direct link to Medication Information and Doctor’s Order Form. Use this form for any and all medications that medically require administration during the school day. Incomplete or incorrect forms will not be accepted. Only one medication per form. Medication will NOT be accepted without a doctor’s order and parent/guardian must sign medication in with the school nurse. Students may NOT bring medication to school. All orders EXPIRE at the end of each school year. Medications that can be given at home are expected to be given at home. 

~A Self Carry order is available for EMERGENCY RESPONSE medication use by students who have received instruction and permission from their doctor: Direct Link to Self-Medication Doctor's Order and Parent/Student Agreement - 1) Self Carry Order- Your doctor will fill in the medication boxes and must also check off the “Medication and/or Procedure” box verifying that your child is competent to use their emergency medication as ordered. 2) Parent/Student Agreement- Parent and student will check off and sign this form and turn in with the Self Carry doctor’s order to the school nurse. The student will report to the nurse with the medication for an initial medication check. The student is responsible for having this medication available during school hours and school sponsored events. {This order is most often used by students in Upper El and Middle School who are ready to take more responsibility for their condition and medication. Staff assistance will be available as needed.}  

Overnight Field Trip Medications

Emergency medications submitted to the school nurse with a doctor’s order will accompany those students on field trips without further action from you. HOWEVER, if your child takes over the counter or prescription medication at home and will require it on an overnight field trip, you will need to obtain a doctor’s order for each medication.Direct link to Medication Information and Doctor’s Order Form. Field trip orders will remain on file for the school year and may be used for all overnight field trips, as long as your doctor has not listed an end date on the order and the medication dosing has not changed. Parents/guardians will be notified at least 4 weeks in advance of all overnight field trips and are responsible for completing the online *Overnight Field Trip Emergency & Medical form* as soon as it is received and submitting a valid doctor’s order and medication to the school nurse at least 2 weeks prior to trip departure. NO medication will be accepted without a valid doctor’s order. Please send only necessary medication on school trips.
